Located at the bottom of the Assessment Extension Flyer for UG/PG is a direct link to apply for extension. Please check your Unit Outline before applying as some Units do not allow assessment extensions. You must get in contact with your lecturer or Unit Coordinator for further details.
Kindly fill out the form to the best of your ability providing the unit code, unit name, and teacher. Please also provide details on the reason for extension. It is imperative that you provide evidence as to why the extension is required.
If your extension relates to an illness or injury or ongoing medical treatment, please provide evidence as to why the extension is required such as a stamped medical document, doctor’s note, or Ministry of Health certificate.
If your extension relates to travel plans or bereavement, please provide an official certificate, proof of flight details, arrangements, and visa (if applicable).
